Fiber to the Home Pre-Installation Survey

We kindly ask that you take a few moments to complete all the questions in this survey before we proceed with your installation. If you have any specific concerns about the installation site, please bring them up with our technician or contact the business office prior to the installation date. What is the E911 address we will be installing the fiber service at?

Would you like this to be a recorded utility easement?
If the answer is yes, a $25 recording fee will apply. This also will require someone schedule to come by the Business office and sign with two witnesses and a notary present.
The fast internet box (NID) is a small box installed at a location where the telecommunications network ends and connects with the wiring of the customer's premises. The NID is also designed to facilitate easy testing of the line by the telecommunications provider to troubleshoot any issues and determine if any problems are on the provider's side or the subscriber's side of the network.

Glenwood Telephone Company shall not be held responsible for any damages to trees, bushes, or other plant life that may occur as a direct result of the placement, installation, maintenance, or operation of the NID. While we make every effort to avoid or minimize any disruption to your landscaping, by accepting the installation, you understand that some trimming or alteration of plant life may be unavoidable.
Please note if your home has existing easements to the Rights of Way we will also need an additional Easement from that property owner.
Is it okay if we dig a little trench to put the internet cable underground?

Please note that Glenwood Telephone Company is not responsible for any damage to briefly-located or non-located utilities that are not marked by the owner during installation.

By accepting our services, you acknowledge and agree that: Pre-Installation Survey: A thorough assessment will be conducted to determine the optimal placement for the NID. We will make every effort to agree upon a suitable location that considers both service functionality and aesthetic considerations. No Liability for Flora Damage: Glenwood Telephone Company shall not be held responsible for any damages to trees, bushes, or other plant life that may occur as a direct result of the placement, installation, maintenance, or operation of the NID. While we make every effort to avoid or minimize any disruption to your landscaping, by accepting the installation, you understand that some trimming or alteration of plant life may be unavoidable. Customer's Responsibility: It is the customer's responsibility to maintain the area around the NID, keeping it clear of any obstructions that could interfere with the access or functionality of the device. Damage to NID from Flora: If plant growth around the NID results in damage to or malfunction of the device, repairs may be subject to service charges according to our standard rates and policies. We appreciate your understanding and cooperation in this matter. If you have any specific concerns regarding the installation site, please discuss them with our technician prior to installation.
